Policy 7140 P1 Administration of Surveys and Data Collection

Data collection and surveys include all paper-pencil and electronic forms of gathering data through a single question or multiple questions and mining data from district owned data sets. All data collection must be ethical and equitable, including versions in English and Spanish; clearly benefit students; not require nor be perceived to require participation; adhere to the Utah Student Data Privacy Act and FERPA; and align with District and Board plans and goals.

Surveys are used by the state, district, and schools to understand multiple viewpoints and perceptions which are used to plan for better school and student programs. The Board encourages stakeholders to respond to state-, district- and school-initiated surveys.

To ensure the District’s goals and requirements are met, regarding surveying, the following general procedures shall apply:

  1. Anyone administering data collections/surveys in Provo City School District will complete a survey registration online to:
    • Identify the targeted population (to prevent over saturation of any given population over any given window of administration)
    • Verify that both content and administration is ethical and equitable
    • Ensure the survey does not violate Utah Student Data Privacy Act and FERPA requirements
    • Ensure content properly reflects the purposes and designs of the district
    • Warehouse data for future reference and comparison 
    • Benefit the district and/or education at large
    • Produce reliable data
    • Ensure questions are ethical, safe, and meaningful 
    • Ensure surveys are administered on a timely basis (to prevent oversaturation)
  2. Persons responsible for administering proposed surveys or data collection will complete a one-time, short, district-approved training video online
  3. Large distribution of surveys will be coordinated with the district’s Director of Communications.
