Student Surveys

Provo City School District wants students to be successful.  Schools with the permission of parents, per House Bill 182 occasionally administer student surveys to know how best to support students. Below is a list of non-academic surveys with sample questions that are asked. Before each survey is administered parents have the right to review the survey questions in their entirety.

The amendment requires schools to annually require written parental consent for certain surveys given to students. Here are some examples of Non-Academic surveys used in our schools.

School Climate Survey

School climate measurement involves a comprehensive assessment of student engagement, school safety, and the learning environment. The measurement of school climate provides educators with the necessary data to identify school needs, set goals, and track progress toward improvement.

Social Emotional Learning Survey

Social-emotional learning (SEL) describes the mindsets, skills, attitudes, and feelings that help students succeed in school, career, and life. At its core, SEL focuses on students’ fundamental needs for motivation, social connectedness, and self-regulation as prerequisites for learning.

Sharp Survey

The Utah Student Health and Risk Prevention (SHARP) survey is the most comprehensive source of information on the challenges and opportunities our youth face. The survey is designed to assess adolescent substance use, anti-social behavior, and the risk and protective factors that predict these adolescent behaviors.

Mental Health Screener

Universal mental health screening can increase the chances of identifying children experiencing behavioral and emotional problems and contributing to improved academic performance and fewer mental health problems in adulthood.
