Policy 5320 P2 Compensation and Benefits: Insurance
Ammissibilità
Contracted employees and their dependents are eligible for Provo City School District benefits on the first day of the month following the date of hire. Eligible dependents include: the employee’s spouse, children under age 26, or disabled dependents of any age. Benefit selections remain in effect until the next open enrollment period or upon qualifying event, i.e., marriage, divorce, birth, etc. The employee must contact the benefits office within 30 days of a qualifying event.
Insurance Review
The District and association representatives will review annually the group insurance policy coverage. Both the association and the District shall agree to any changes in coverage and/or adjustments to meet current medical and hospital charges or needs, before any changes will be effected. Once the extent of the coverage has been agreed to, the decision of whom the insurance carrier shall be will be decided by the Board of Education.
