Policy 5120 Employee Accident/Injury Reporting Policy
In an effort to maintain a safe and healthy work environment, accidents that occur on Provo City School District property or during the course of employment with the District must be reported. It is the intent of the District to minimize accidents, injuries, and illnesses by correcting identified causes when appropriate and feasible.
The Employee Accident/Injury Reporting Policy and Procedures apply to all accidents involving District employees. All employees of the Provo School District are covered by Workers’ Compensation Insurance through the Utah School Boards Association. Failure to comply with the Employee Accident/Injury Reporting Procedures (5120 P1) may result in a denial or partial denial of the accident claim with the Workers’ Compensation Insurance.
Fraud
5120 — “Under Utah law, a fraudulent workers’ compensation claim for compensation or medical benefits is a crime and any employee found guilty of fraudulently receiving these benefits is subject to fines and incarceration.”
