Transition Services Guide

Throughout your child’s schooling both you and they have gone through many transitions. In Provo City School District as well as other districts a student may transition from preschool to kindergarten and then to elementary, next to middle school, and onto high school and potentially post high school. When your child completes their schooling, you and your child should celebrate their accomplishments and the transition to adulthood.

Perhaps the most challenging transition is from school to adult life. In preparation for leaving Provo City School District, we plan to assist your student to go onto further education and training, employment and as needed independent living. The purpose of IDEA, the law that governs Special Education services, is to ensure that all children with disabilities have available to them a free appropriate public education (FAPE) that emphasizes special education and related services designed to meet their unique needs and prepare them for further education, employment and independent living. This process begins in preschool and goes throughout their years in public education.

The Transition Guide has resources and information that will help in this process. Your students IEP team are essential to this preparation. Consult with them as needed throughout this process.

Provo City School District Special Education has developed the Transition Guide to assist Parent’s, Student’s, Educational staff, and Community member’s to better prepare students for post secondary life experiences.
