Employment Applications
These are the only application forms that Provo City School District will accept for prospective employees. Do not submit any other employment application forms.
The employment application forms are in Portable Document Format (PDF). To view and print them, you must have Adobe Reader installed on your computer. Adobe Reader is a free download.

To fill out an application by hand
- Download an employment application form by clicking one of the links above.
- Open the employment application using Adobe Reader.
- Print the application form.
- Fill the application out by hand. Don't forget to sign and date it at the bottom.

- Type your information into the application. (you will not be able to save the application).
- Print the application once you have filled in all of your information.
